When an observation is marked as an “urgent safeguarding concern,” it instantly triggers a Live Risk Assessment Log with mandatory fields for Risk Observed, Action Taken, and Next Steps. This log is then permanently linked to both the original observation and the resident’s profile, creating a clear, audit-ready trail from concern to resolution.

Click any entry to open its full details. From a Risk Assessment Log, you can navigate directly back to the original observation for full context. Need to find something fast? Use the “Filter risk log” search with keywords to surface all related entries across the centre or within a specific resident’s history.

Every resident has their own safeguarding space on their Profile Page, mirroring the overview layout but focused solely on their journey. It automatically includes their Initial Risk Assessment and every subsequent log and incident, providing a complete chronological record for assessments, planning, and reviews.

How it works — a quick walkthrough

  • Click "Safeguarding": To navigate to the main Safeguarding page for a centre-wide view.
  • Filter your view: Select your preferred date range and toggle between ‘Open’, ‘All’, or ‘Resolved’ risk logs.
  • Dig deeper: Click the arrow on any log to open it fully, then click again to view the linked original observation.
  • Find fast: Use the keyword filter to search across all logs.
  • Focus on a resident: Open any resident’s profile and select the Safeguarding tab to see their complete, individual history.
